Norwalk Community Health Center (NCHC) is a Patient Centered Medical Home (PCMH).
As a patient, you have a team of health and wellness professionals, led by your physician, working with you to achieve your best health.

Everyone who needs our services is welcome.

No one is turned away because of inability to pay.



  • + Our flagship location is at 120 Connecticut Avenue, Norwalk across from the Norwalk Fire Department (Google Map).
  • + Our first satellite location is at 55 Chestnut Street in So. Norwalk at Smilow Life Center (Google Map).
  • + Our sunshine yellow Medical Mobile Unit is out and about in the community, to bring information, health, and wellness services where they are needed most and to community events.

Norwalk Community Health Center is a nonprofit health care provider. Since 1999, generations of men and women throughout Fairfield County and beyond have entrusted their pediatric, adult, women’s/OBGYN, dental and behavioral health care and the well-being of their children and families to us. We take that trust seriously. Norwalk Community Health Center is rated among the top community health centers in the nation. We are dedicated to maintaining the high quality of care we provide our patients – from newborns to seniors – that enriches the lives at every stage and leads our community toward achieving its best possible health.


  • + As our patient, you have a provider-led care team in your corner. You receive the benefit of cross-department care teams working together with you to reach the best health outcomes.
  • + Free wellness events and group programs are offered for our patients. We partner with local agencies and groups to bring health education and services to the community and to our patients.
  • + We accept most insurances. And most importantly, no one in need of our services is turned away because of status or inability to pay.

Tues: 8:00am to 8:00pm

Wed: 8:00am to 8:00pm

Thur: 8:00am to 8:00pm

Fri: 8:00am to 6:00pm

Sat: 8:00am to 12:00 noon

Not all services are available after 5:00pm or Saturdays.
